I believe it's a mint error, if it were struck outside of the mint by something the devices would be flattened and wider, take the columns for instance they look normal to me, so I think it was done at the mint

I just did, doesn't look like there is a match!
Think mine is a dual clash as well, but doesn't line up with the one on maddieclashes

Just found this one, the reverse must have been changed out because there are no signs of clashing or die polishing.

Die break for the fatter looking one and just a radial die crack for the one running to the rim.
If the fields were shifted on each side of the crack or a break somewhere along that crack then it would be called a Radial die Break
